Alan Shatter's Fine Gael today released a confidential HSE investigation into the death of a girl '(T''). It reveals an appalling litany of failure on the part of the State dating back years. Again and again the State had the chance to save this girl, and again and again it failed. Again and again it did not implement it's own child protection protocols.

But has any social worker being named in this report? Will any be forced to resign? Not in a million years. It is only when Church personnel fail children that we get widespread calls for resignations. Please explain.
